The Jackson County Sheriff's Department and the Jackson City Police Department have both faced reductions of staff and increases in call service requests over the past few years. Both agencies have been forced to review the types of services that are provided to the Jackson community. 

In an cooperative effort to provide the best police services to the community, a uniform self reporting private property damage accident form was developed. The form was designed with the user in mind. 

The Private Property Accident Report form is available at both agencies, shopping malls, customer service desks, and larger grocery outlets. After completion of the form, the form is either mailed to or dropped off at the respective agency.

Then, fill out the form and then send it to your printer, making 3 (THREE) COPIES (ONE FOR YOU, ONE FOR YOUR INSURANCE, AND ONE FOR THE SHERIFF DEPT. FILES). Finally, either mail or drop off the form to the Jackson County Sheriff's Department at 212 West Wesley Street, Jackson, Michigan, 49201.
